Proofread + update KBreakout handbook to 16.04

remove entities kbreakout, kappname, package - defined in kdoctools or not used anymore
bump date + releaseinfo
add screenshots
remove appendix
replace kappname with kbreakout
fix wrong shortcuts
add info about Fire on mouse click
