      Thanks to Eric Newberry (from Google Code-In) for running the heavy
      lifting on this.
      
      I made some other corrections since we're touching all the source
      anyways (e.g. fixing my email addresses), fixing the encoding of Richard
      Lärkäng's name, and I've also chosen to avoid the fancy box-shaped
      comments so that there's no issue with trailing spaces/tabs.

      * Remove support for the cover system used in KDE 3.3.
      * Cover dragging uses "pure" Qt4 now.  Unfortunately the cover manager is
        still Qt 3 so drag-and-drop from there is temporarily disabled.
      * Add a coverId() function that ensures the m_coverId for a CoverInfo is
        properly set before use.

      Make random play work better.  Before only the old style unused random list was
      being updated when a playlist changed.  In addition, there are a lot of
      improvements to the new cover support.
      * You can drag-and-drop covers onto playlist items.  Right now only selected
        items are altered even if you drag onto a
        different item, so I'll have to change that.
      * Clearing a cover from a track no longer removes it from every track using
        that cover.
      * Replacing a track's cover works again as well.
      
      I'm sure that both features could use more work so I guess it's back to the
      grindstone for me, I just wish I had more time. :(

      import covers stored in the original format, so starting up JuK for the first
      time with this code will probably take awhile, after that the speed should be
      unaffected.
      
      The CoverManager is capable of associating a track with an arbitrary cover.
      Each cover in the CoverManager can have associated with it an Artist and
      Album to allow for automatically applying covers to newly added tracks.
      However now the cover for a track isn't internally limited based on its
      Artist and Album info.
      
      Note that this needs some fleshing out.  The GUI will definitely need some
      improvement, and some things may not work exactly like they used to, but at
      least we've got a good base under us now.

      GUI: Overhaul the file renamer configuration dialog based on a design of Scott's.  This involved a lot more code than I thought at first, but it should be pretty neat.
      
      This also should fix wish 64849 (Add genre to file renamer) and wish 63912.
      In essence this creates a dialog that has a row with each possible category, and arrows allowing you to move that category up and down.
      * In the options for the category, you can control leading and trailing text, and what happens if the tag is empty.
      * You can set a minimum width for the track (bug 63912).
      * The dialog includes a sample result, and you can either edit the sample tag values manually, or load tags from a file to see what it would look like.
      * Right now the file renamer code automatically suppresses separators between tag values that use bracketing characters, since I feel that stuff like Artist - Title - [Track] looks dumb, but that may be changed/removed later.
