Keysmith rebranding

Johan Ouwerkerk requested to merge ouwerkerk/otpclient:keysmith-rebranding into master

Renaming exercise: excise mentions of "otpclient" and call everything Keysmith/keysmith instead.

This MR builds on top of !15 (merged)

Note: this changeset should be mostly uncontroversial, the only issue I see is whether or not to capitialise "Keysmith" when used as QApplication or QSettings name parameters.
