Restructure initialisation a bit.

 - Do important card init in firstopen
 - Give each channel it's own cmdbuf dma object
 - Move RAMHT config state to the same place as RAMRO/RAMFC
 - Make sure instance mem for objects is *after* RAM{FC,HT,RO}
