Misc UI/UX changes

Joshua Goins requested to merge work/redstrate/misc-changes into master
  • Remove the duplicative instance name from the account switcher
  • Don't allow "managing" the following/followers of other accounts
  • Improve the cross-action dialog by adding the account's avatar and other info
  • Improve the functionality of the "More" menu on the notifications page, make it look less buggy.
