      This removes the singleton and a bunch of statics initialization
      code that were spread through the code base.
      
      Also, a calendar isn't needed now, which speeds up things and
      saves memory if you have lots of incidences.
      
      counter proposals now almost work, they show the incidence editor
      only no e-mail is sent yet, a fix for that will go into kdepimlibs.

      At that point the incidence object the alarm was linked to doesn't exist
      anymore... So we instead use a custom property set by the ETMCalendar
      (that's part 2, committed in kdepimlibs).

      We need to make sure all the collections are enabled, otherwise we'll
      never get to see any item in the calendar. I wonder if that ever worked,
      to me it looks like korgac could have been broken since the introduction
      of the KCheckableProxyModel.
      
      It is a rather crude implementation as we blindly enable all the
      available collections here. We might want to improve that later by
      enabling only a subset based on some settings (either specific to korgac
      or in sync with korganizer). That's to open up that path that I didn't
      simply use setCollectionFilteringEnabled(false) on purpose.

      Previously, the list of already-triggered reminders only got saved to
      the config file when the user actively dismissed or suspended one of
      them. With this and the previous patches, saving and loading of
      reminders should work correctly now.

      We need to wait for all collections to be available when restoring
      reminders at startup, or they won't get loaded properly. Additionally,
      we must delete reminders one by one as they are dismissed instead of
      just cleaning out the config file after each startup.

      Wait for all collections to be populated before performing checks for reminders. Avoids the situation where KOrganizer thinks it has already checked for outstanding reminders in all collections even though it only looked at an uninitialized, empty list of collections.
