Fixes to new effects layout

Fixed broken links, table wrap, missing parameters/text, and added some minor updates. Added the new how-to for Lift, Gamma, Gain, and the new alphabetical lists of video and audio effects and compositions (need more work in the main section).

Every document I touched for the fixes has now the |icon| definition for any icon used. This is in preparation to eliminating the |icon| definitions in the conf.py file for performance reasons.
