Minor README polishing

Gitlab renders the README nicely if it's markdown/rst or something similar.

This PR renamed README -> README.md, and tweaks the formatting a bit so that it renders nice on GitLab. Also updated the bug reporting URL.

Rendered version can be previewed on the fork I've made for this PR.
