update player toolbar

Various code cleanups and modernizations.

Also reorder the buttons. Before:

kwave-standard-icons

After:

kwave-reorder-playback-toolbar

This groups the play, pause, and stop buttons together, and groups the back and forward buttons together at the end. That seems more logical to me, any thoughts?
