Move "Add" actions one level up

Arjen Hiemstra requested to merge work/ahiemstra/add_placement into master

This moves the "add row/add title" actions to the page toolbar, moves the "add column" into the row controls and "add section/add separator" to column controls. This way all the actions affect only the thing they are attached to.
