Add Windows version info and icon to QCacheGrind built with qmake

Common version information is put into an include file.

The icon is generated from running convert on kcachegrind/*.png. Notably, a 16px icon is missing, but the downscaled icon doesn't look too bad.

I'd like to do a pass on Mac stuff, but I don't have one yet.
