Building towards default/advanced token parameters UX + otp URI support

MR to collate various follow-up changes to other MRs I'm working on:

  • I want to re-work the UX for adding accounts, specifically to filter out "required" from "advanced" options and avoid cluttering the UI for one token type with irrelevant details about another type.
  • A natural extension point of this UX is to support a split between "manual" and "scanned" tokens later
